These terms and conditions shall supersede any subsequent terms or conditions included with any purchase order, whether or not such terms or conditions are signed by SDP Dental Plan Inc. SDP Dental Plan reserves the right to make changes to this site and these terms and conditions at any time.
Cancellation Policy
Membership fees are refundable within five (5) days of your enrollment. Requests must be made by phoning us at 1-800-476-4316. Following the five-day period, requests must be received in writing and, if approved, there will be a 30% processing fee and monthly prorate. If the plan is utilized within the cancellation period and discounts are received the cancellation period is void and no refunds will be given. All plans require a one-year minimum membership. Any additional years purchased can be refunded before that year of membership begins, and the price of time used will be adjusted accordingly. The $19 registration fee is non-refundable. All monthly plan options require a one-year contract; after the full year is completed, monthly payments can be stopped at any time.

Typographical Errors
In the event a product is listed at an incorrect price or with incorrect information due to typographical error or error in pricing or product information received from our suppliers, our Dental Plan shall have the right to refuse or cancel any orders placed for product listed at the incorrect price. Our Dental Plan shall have the right to refuse or cancel any such orders whether or not the order has been confirmed and your credit card charged. If your credit card has already been charged for the purchase and your order is canceled, SDP Dental Plan shall immediately issue a credit to your credit card account in the amount of the charge.

Order Acceptance Policy
Your receipt of an electronic or other form of order confirmation does not signify our acceptance of your order, nor does it constitute confirmation of our offer to sell. SDP Dental Plan reserves the right at any time after receipt of your order to accept or decline your order for any reason. SDP Dental Plan reserves the right at any time after receipt of your order, without prior notice to you, to supply less than the quantity you ordered of any item. We may require additional verifications or information before accepting any order.
In the event that your initial membership order is paid via Regular U.S. Mail in the form of a paper check or money order, it is the responsibility of the member to supply SDP Dental Plan with current payment information to process membership renewal. Payment options are limited to the following: credit card, electronic check withdrawal, or paper check or money order sent Regular U.S. Mail. In order for your membership to continue without interruption, your renewal payment must be received two (2) weeks prior to the expiration date identified in your order confirmation. In the event that our Dental Plan attempts to renew your membership but is unable to do so due to the cancellation or inactivation of your credit card or other form of electronic payment, our Dental Plan will attempt to contact the member through the e-mail address or phone number on record. In all events, it is the responsibility of the member to ensure that our Dental Plan has received payment for membership renewal two (2) weeks prior to the expiration date identified in the order confirmation. SDP Dental Plan is not responsible and assumes no liability to any member for any member’s failure to provide our Dental Plan with current payment information for membership renewal. By electronically accepting the terms and conditions of SDP Dental Plan order acceptance policy, the member agrees and understands that he/she is entering into a binding and enforceable legal contract.
Sales Taxes
SDP Dental Plan shall automatically charge and withhold the applicable sales tax for orders to be delivered to addresses within New York. Each customer shall be solely responsible for all sales taxes, or other taxes, on orders shipped to any other state.
Jurisdiction and Venue
You agree that any legal action brought against our Dental Plan shall be governed by the laws of the State of New York without regard to its conflict of law principles.
